School-based flu shots @ BEA
BEA Schools are partnering with United Hospital District Clinics to provide flu shots to our students in Kindergarten through Grade 12. Shots will be given at school, during the school day on Friday, November 1.
Want your child to participate? Complete, sign and return a consent form to the school office by October 31. Blank consent forms were sent home with all students this past week, but you may also print consent forms from this link, or from the BEA Schools website.
Important update: United Hospital District recently upgraded their documentation and patient management systems. As a result, the following two documents are also required in order for students to receive flu vaccine at school: New Patient Form and Statement of Financial Responsibility and Release of Information.
If you would like more information on the flu vaccine, please see the CDC’s Vaccine Information Statement.
Flu vaccination is the best tool we have in our toolbelt as we work to fight the flu!
